On this page you will find downloadable consent forms for both of our activities. A signed consent form is required before playing. Completing these prior to arrival helps speed things up on the day.

Minors (anyone under the age of 18) MUST have their consent form countersigned by a parent or legal guardian. If parents are not planning on attending the event, then it is essential that the consent form is signed and submitted online OR signed and sent with their child on the day of the event.

Upon making a booking over the phone a member of our team will ask you for your email address, so that we may send you a link to submit the consent form online. This link can be forwarded to others if you are booking for more than one person.

 

Alterantively there are downloadable copies of our consent forms below which you can print out and bring along with you on the day of your event. Please make sure you fill out the correct form and fill in all of the information clearly. Remember, anyone under the age of 18 must have their consent form countersigned by a parent or legal guardian!
